For your convenience I have included an order form to use as either a work sheet to plan your order, or to complete for ordering purposes if you have your mind set. If you have print capability, print it, fill it out and have it handy when we talk. If you choose to send to me by regular mail be sure to include your phone number and I will personally call you to review your order.
